✅ 1. For FSSAI Basic Registration

(For small businesses with turnover ≤ ₹12 Lakhs/year)

• Passport-size photo of the applicant

• Government-issued Photo ID (Aadhaar, PAN, Voter ID)

• Business name and address proof (utility bill, lease/rent agreement)

• Nature of food business

• Declaration form (Form-A)

✅ 2. For FSSAI State License

(For businesses with turnover ₹12 Lakhs to ₹20 Crores/year)

• Form-B duly completed and signed

• PAN Card of the applicant or company

• Passport-size photo and ID proof of proprietor/partners/directors

• Address proof and layout plan of the food business premises

• Rent agreement or ownership proof

• List of food products to be handled

• NOC from the local municipal authority

• Water testing report from a recognized laboratory

• Partnership deed / MoA & AoA / Incorporation Certificate (as applicable)

• Food Safety Management System (FSMS) Plan or certificate

• Valid email ID and mobile number

✅ 3. For FSSAI Central License

(For large businesses with turnover > ₹20 Crores/year or businesses involved in import/export)

• All documents required for State License

• Import Export Code (IEC), if importing/exporting food

• Certificate from Ministry of Tourism (for hotels)

• NOC from FSSAI (if applicable)

• Proof of turnover (audit report or sales certificate)

• Source of raw materials for milk, meat, etc.

• Pesticide residue report (for packaged drinking water or bottled water units)
